Analysis of Materials Utilized in Frontalis Suspension Surgery for Ptosis Correction: A Network Meta-Analysis

Purpose
to evaluate different materials employed in frontal suspension ptosis surgery, specifically focusing on ptosis recurrence rates.
Methods
This review was conducted with the guidance of PRISMA 2020. A literature searching was done in PubMed, Scopus, and Web of Science. The primary outcome of this review was the ptosis recurrence following the repair between the material. Due to more than 2 materials being collected in the preliminary searching, a random-effect frequentist network meta-analysis (NMA) framework was generated. Ranks among materials were analysed using P score.
Results
Eight studies were included in this review with 8 different materials identified consisting of polytetrafluoroethylene (PTFE), fascia lata, Gore-tex, nylon, polypropylene, ethibond, silicone, and silicone-ethibond mix. By comparing other materials to fascia lata, the odds ratio (OR) of ptosis recurrence was estimated. Nylon was the sole material that significantly increased ptosis recurrence (OR 4.06; 95%CI 1.30 – 12.65) whilst the rest did not. Although PTFE did not show any significant difference with fascia lata (OR 0.90; 95%CI 0.26 – 3.04), PTFE was the 1st ranked material in reducing chances of ptosis recurrence according to P-score analysis (P score 0.83), for general age range of patients.
Conclusion
To mitigate the risk of ptosis recurrence, no single material stands out as the best option. Nevertheless, PTFE and fascia lata demonstrate the lowest recurrence rates. In contrast, nylon has been linked to a higher likelihood of recurrence, as indicated by the network meta-analysis (NMA), suggesting that its use should be avoided.
