Causal association between lifestyle behavior and potential risk factors with blepharoptosis

Purpose

Blepharoptosis is among the most common disorders of the eyelid malposition that can affect appearance and impair visual function, both of which can negatively impact quality of life. Various type of blepharoptosis have been reported, neurogenic, traumatic, congenital, mechanical, psychogenic and myogenic, while in about 15~25% remained uncertain. We aim to identify targets for the prevention of blepharoptosis and its associated disease burden.


Methods

We used the 2-sample Mendelian randomization method to investigate the causal associations between reported potential risk factors and lifestyle behaviors with blepharoptosis. From European-descent genome-wide association studies, we selected genetic variants (P<5×10−8) for prediabetic status, obesity parameters, smoking, alcohol and coffee consumption, hypertension, education level, physical activity, and sleep duration. We extracted the genetic associations with blepharoptosis from the FinnGen Study (1 349 cases and 203 231 controls). Inverse-variance weighted was mainly used for statistical analyses.


Results

Genetically predicted leisure screen time (pooled odds ratio [OR] per 1 SD, 1.79 [1.23– 2.60]) was associated with a higher odd of blepharoptosis. Suggestive evidence was obtained for higher BMI and current smoking with a higher blepharoptosis odds, physical activity and smoking cessation with a lower blepharoptosis odds. No association of other genetically predicted factors was observed.


Conclusion

Mendelian randomization study identified current smoking status and body mass index as causal risk factors, and smoking cessation as protective factors for blepharoptosis. This implicates that these modifiable risk factors are important targets in the prevention of blepharoptosis.
