Transconjunctival Triamcinolone Injection for Upper Eyelid Retraction in Thyroid Eye Disease: Changes in Marginal Reflex Distance 1 and Clinical Activity Score

Purpose

To evaluate changes in marginal reflex distance 1 (MRD1) and clinical activity score (CAS) after transconjunctival triamcinolone (TA) injection for upper eyelid retraction in thyroid eye disease (TED).


Methods

A retrospective review was conducted on the medical records of TED patients who received one or more TA injections for upper eyelid retraction. Treatment outcome measures included MRD1 and CAS, with all seven CAS parameters assessed before and after treatment. Treatment success was defined as an MRD1 improvement (≥0.5 mm decrease) and a reduction in CAS.


Results

Of 250 eyes evaluated, MRD1 improved in 74.8% (n=187 eyes), with a significant reduction from 5.0 ± 1.6 to 3.7 ± 1.4 (P < 0.01). CAS improved in 50% (n=125 eyes), decreasing from 1.2 ± 0.8 to 0.6 ± 0.8 (P < 0.01), and all seven CAS parameters showed significant improvement following TA injection (all P < 0.05). The mean number of injections per patient was 1.8 ± 1.0, and the mean dose of injections was 39.4 ± 25.4 mg. Treatment outcomes were assessed 17.9 ± 16.3 months after the final injection, and the mean follow-up period was 23.0 ± 19.9 months.


Conclusion

Transconjunctival TA injection is an effective treatment for improving MRD1 and reducing CAS in TED.
