Purpose
To analyze the clinical features, management, and survival outcomes of patients in whom orbital metastasis was the first sign of undiagnosed malignancy.
Methods
This retrospective case series (2014–2022) included 10 patients (33.3% of 30 orbital metastasis cases) without prior cancer diagnosis. Data on demographics, clinical presentation, imaging (PET/CT), histopathology, treatment, and survival were analyzed. Ethical approval was obtained, adhering to the Declaration of Helsinki.
Results
The cohort had a mean age of 68 years (range: 52–77), with female predominance (60%). Lung (40%) and breast (30%) were the most frequent primaries. Proptosis (90%), diplopia (60%), and ocular pain (50%) were common presenting symptoms. Systemic evaluation revealed disseminated disease (mean metastatic sites: 3.3). All patients received chemotherapy; 40% underwent radiotherapy, and 30% had primary tumour surgery. Mean survival was 5.8 months, significantly shorter than patients with known malignancies (12.3 months; p<0.005).
Conclusion
Orbital metastasis as the initial malignancy sign correlates with aggressive disease and dismal survival, particularly in undiagnosed cases. Rapid-onset proptosis, diplopia, or orbital pain warrant urgent systemic evaluation, including PET/CT, to identify occult primaries. Multidisciplinary management is critical, though prognosis remains poor, highlighting the need for public health initiatives to improve cancer awareness and early detection in regions with limited healthcare access.
