HYPOFRACTIONATED STEREOTACTIC RADIOTHERAPY FOR ORBITAL METASTASES: FEASIBILITY, EARLY OUTCOMES, AND PALLIATIVE EFFECTS

Orbital metastases are relatively uncommon and can cause symptoms as adnexa response, proptosis, relative afferent pupillary defect, diplopia, decreased visual function leading to pain and reduced quality of life. Often are lately detected or overlooked due to a number of symptoms of disseminated oncological disease and various side effects of modern systemic therapy. When intraorbital lesion is detected, it’s very often not available for biopsy. Surgical treatment isn’t an optimal treatment modality considering extended malignant disease and mutilating effect.
Methods
Eight patients received stereotactic radiotherapy. Stereotactic treatments were performed using linear accelerator Varian EDGE and CyberKnife. Treatment was delivered in 1–10 fractions.
Results
Radiologic response was evaluable in six patients. Partial response was observed in five, and complete response in one patient. Significant clinical improvement were noted in all patients, including reductions in periocular edema, proptosis, diplopia. Patients reported an overall better subjective well-being.
Conclusion
Stereotactic radiotherapy offers an effective approach to control orbital disease, alleviate symptoms, and preserve vision. Achieving good local control of the orbital disease with minimal side effects is tasking stereotactic radiotherapy as a first line of treatment. The treatment is non-invasive, safe and effective. It is known that a more aggressive biological type of tumor represent itself through the intraorbital metastases, so it’s necessary to refer the patient to stereotactic radiotherapy in a timely manner without wasting time on additional diagnostics.There are no financial or non-financial competing interests to report.Reference: Palmisciano, P, Ferini, G., Ogasawara, C, Wahood, W., Alamer, O.
