Purpose

To evaluate safety and early outcomes of a novel ultra-high molecular weight polyethylene (UHMWPE) orbital implant with an 8-tunnel suturing platform (EZYPOR) in primary and secondary enucleation.

Methods

This single-center retrospective case series included consecutive patients who underwent enucleation with EZYPOR implantation from January 2021 to March 2026, with a minimum follow-up of 6 months. Outcomes included implant safety (exposure, migration, infection, conjunctival thinning), prosthetic motility, and aesthetic restoration based on orbital volume and eyelid symmetry relative to contralateral eye.

Results

Twenty-eight patients (25 primary, 3 secondary enucleation) were included. No implant exposure, extrusion, migration, infection, or clinically significant conjunctival thinning occurred. All cases achieved stable prosthetic motility in primary and cardinal gaze. Aesthetic outcomes were satisfactory in 89.3% of cases (25/28). Three cases with prior trauma or lid laxity required reduced implant sizing to optimize tension-free closure.

Conclusion

The EZYPOR UHMWPE implant demonstrates strong early clinical outcomes in enucleation reconstruction. Its 8-tunnel suturing platform enables precise, reproducible extraocular muscle fixation without wrapping, minimizing technical variability and enhancing consistency of prosthetic motility. Compared with conventional porous implants, UHMWPE provides a structurally robust alternative with favorable handling and potential cost efficiency in resource-limited settings. These findings suggest a promising step toward standardizing muscle fixation in socket reconstruction, warranting prospective comparative evaluation with objective motility metrics and long-term outcomes.
