Extensive Sebaceous Cell Carcinoma of the Upper Eyelid: 1-Step Reconstruction in a Young Male

Sebaceous cell carcinoma (SCC) is a rare, aggressive, and difficult-to-diagnose neoplasm that primarily affects elderly women and occurs most commonly in the upper eyelid. It presents as a slow-growing, painless lesion and is often mistaken for chronic blepharitis or chalazion. A 44-year-old male patient presented with a solid yellowish tumor measuring 20 x 15 mm on the right upper eyelid, associated with madarosis and loss of normal eyelid architecture. He had previously undergone six surgical procedures, with subsequent local recurrence of the lesion. In addition, previous histopathological examinations had yielded discordant results, with the most recent examination being consistent with SCC. Complete excision of the lesion was performed in association with intraoperative frozen-section biopsy. Initial evaluation demonstrated involvement of the nasal margin, requiring enlargement of the surgical margins, followed by repeat histopathological assessment, which showed no evidence of residual involvement. Eyelid reconstruction was performed using a tarsoconjunctival graft from the contralateral upper eyelid and an ipsilateral orbital rim periosteal flap to reconstruct the posterior lamella, and an ipsilateral lower eyelid myocutaneous flap for reconstruction of the anterior lamella. The patient evolved with an excellent cosmetic and functional postoperative outcome, despite the presence of some facial hair transferred along with the myocutaneous flap. This case highlights the impact of delayed diagnosis of SCC, the importance of histopathological examination in the presence of any eyelid tumor, and the challenges involved in eyelid reconstruction following extensive resections.
