The Figueiredo Technique for Lower Eyelid Reconstruction After Malignant Lesion Excision: A Case Report

Methods
A 56-year-old man presented with a progressively enlarging right lower eyelid lesion with 5 years of evolution. Excisional biopsy with 2 mm safety margins and full-thickness lower eyelid resection was performed under local anesthesia. Reconstruction employed the Figueiredo technique, consisting of a polypropylene prosthesis fashioned from sterile saline bag material, trimmed to the exact defect dimensions, and sutured into the surgical defect to promote secondary-intention healing in a protected moist environment
Results
Histopathology revealed invasive moderately differentiated squamous cell carcinoma with free surgical margins. Postoperative recovery was uneventful, without infection, keratitis, or need for reintervention. Partial dehiscence of posterior lamellar sutures occurred on postoperative day 14, requiring prosthesis removal. Progressive healing with satisfactory eyelid closure, preservation of ocular surface protection, and favorable aesthetic outcome was observed.
Conclusion
The Figueiredo technique appears to be a safe, reproducible, and low-cost alternative for selected periocular defects after malignant lesion excision. Its technical simplicity and satisfactory functional and aesthetic outcomes make it a promising reconstructive option. As it can be performed under local anesthesia, it may be especially valuable for elderly patients, individuals with multiple comorbidities, or those with contraindications to general anesthesia.
