Brow Fat Pad Repositioning in Transblepharoplasty Browpexy: A Simple Modification to Prevent Skin Dimpling

Purpose

Internal browpexy performed through an upper blepharoplasty incision is effective for lateral brow support, but skin dimpling and contour irregularity may compromise the aesthetic result. We present a transblepharoplasty browpexy modification based on superior repositioning of the brow fat pad as an independent unit to reduce dimpling and improve contour.


Methods

After standard skin-muscle-fat excision as indicated, a bloodless plane is developed between the orbicularis and the brow fat pad. Over the superolateral orbital rim, preperiosteal dissection is performed while preserving the periosteum, allowing superior mobilization of the brow fat pad on a vascularized pedicle. The fat pad is then fixated to the frontal periosteum with 4-0 Vicryl, aiming to provide lateral brow support and volume restoration without tension on the skin.


Results

The technique provided improvement in lateral brow position and temporal hooding, with a smoother brow-eyelid transition and preservation of temporal fullness. In the presented series, visible postoperative skin dimpling was minimized and brow contour appeared more natural, without obvious overcorrection.


Conclusion

Treating the brow fat pad as an independent anatomical unit may offer both functional support and aesthetic volumization during internal browpexy. This modification appears to be a practical adjunct to upper blepharoplasty, particularly in patients with lateral brow descent, and may help reduce one of the main aesthetic drawbacks of classic internal browpexy.
