Delayed adverse events of hyaluronic acid filler and effects on lower blepharoplasty

Purpose
To report on a novel classification of periorbital adverse events associated with hyaluronic acid filler injections, and the subsequent intra-operative and post-operative effects of filler on lower blepharoplasty surgery.
Methods
Retrospective case series of patients with a past history of hyaluronic acid filler injections to the lower eyelid/tear trough or cheek, who underwent lower blepharoplasty surgery performed by 3 surgeons (RM, BP, SSD). Data collected included demographic information, aesthetic history, pre-operative features, additional treatments required and intra-operative findings, and post-operative course.
Results
A total of 50 cases were included. Common patterns of post-filler presentations in this patient population were identified. In preparation for lower blepharoplasty, additional peri-operative treatments were often indicated, most commonly hyaluronidase for dissolution of filler. A subset of patients experienced prolonged post-operative swelling, requiring further intervention and follow-up.
Conclusion
A novel classification of delayed adverse events from hyaluronic acid filler in the tear trough and cheek is presented: 1) prominent fat pads, 2) sausage-shaped swelling, 3) skin discolouration, and 4) malar oedema. The presence of filler in this region has a marked effect on lower blepharoplasty surgery both intra-operatively and post-operatively, and early identification and anticipation of these issues is critical for surgical outcomes.
