Tailored Surgical Approaches for Complex Eyelid Retraction

Purpose

To describe 4 clinical cases of severe upper and lower eyelid retraction treated with different surgical techniques.


Methods

1st case: A 35-year-old man with cicatricial upper and lower eyelid retraction and lagophthalmos, and a contralateral blind eye, after excision of Kaposi Sarcoma.2nd case: A 16-year-old boy with severe bilateral lateral canthus dystopia and lower lid retraction, causing lagophthalmos and ocular surface exposure.3rd case: A 16-year-old boy with total absence of the anterior lamella of the left upper eyelid, caused by a severe systemic fungus infection with periorbital involvement.4th case: A 24-year-old woman with corneal perforation due to cicatricial lower eyelid retraction and lagophthalmos after a facial burn.


Results

1st case: Underwent midface lift and lower and upper skin grafts.2nd case: Treated with bilateral midface lift, palate spacers and canthal fixation with drill hole fixation.3rd case: After medical treatment and infection control, received a skin graft from supraclavicular tissue and cul-de-sac reformation.4th case: Managed with a skin graft from retroauricular area and superior eyelid botulinum toxin injection.


Conclusion

All cases resulted in improved cosmesis and resolution of corneal disease. Different techniques are required to correct eyelid retraction, and a combination of different procedures is often needed in order to achieve long lasting results.
