Primary Cutaneous Mucinous Carcinoma of The Eyelid Associated with BRCA2 mutation

Purpose

To report the association between primary cutaneous mucinous carcinoma of the eyelid with BRCA2 mutation.


Methods

Case report


Results

A 68 years old gentleman complained of 3 months history of left lower eyelid rapidly growing pigmented lesion which had bled spontaneously. There was no similar lesion elsewhere. He was previously tested positive for BRCA2 mutation but no malignancy found. There was a strong predilection for BRCA2 mutation and the associated malignancies in his family. On general observations, he was alert and pink on air. Physical examinations raised no suspicion of malignancy; no loss of weight or appetite, no palpable lymph nodes, lungs was clear and no palpable abdominal masses. His best corrected vision was 1.0 on the right and 0.60 on the left. There was a broad based pedunculated pigmented lesion measuring 5X3mm with evidence of recent bleeding on the his left lower eyelid laterally, sparing lid margin and lash line. It was neither tender nor inflamed, had poorly defined edges, soft consistency and not tethered to the underlying structures. Early excision biopsy with histopathological examination (HPE) was performed via uncomplicated block excision and direct reconstruction. HPE result came back as mucinous carcinoma of the eyelid. Patient showed no sign of recurrent at 3 months postoperatively and continued on yearly outpatient follow-up.


Conclusion

Primary cutaneous mucinous carcinoma is a very rare type of malignancy and to our best knowledge, the occurence of the tumour on the eyelid was not previously reported in a BRCA2 mutation carrier.
