Surgical reconstruction of post-traumatic anophthalmic orbit

Purpose
To present the plastic reconstruction of a post-traumatic anophthalmic orbit using a Medpor graft for the orbital floor, an intraorbital hydroxyapatite-Medpor artificial sphere graft, and an autologous dermis-fat graft.
Methods
A 22-year-old patient with a post-traumatic anophthalmic left orbit due to a car accident presented to our clinic 6 months later for the correction of severe orbital deformity. The patient had suffered fractures of the orbital floor and medial wall, globe rupture, and underwent initial enucleation due to severe soft tissue injuries, with the eyelids preserved. Ophthalmologic examination revealed an anophthalmic left orbit with eyelid ankylosis and inability to close the eyelids. Imaging showed bone defects in the orbital floor and medial wall with residual soft tissues. Surgical planning included reconstructing the orbital floor with a Medpor graft, replacing the globe with an intraorbital hydroxyapatite-Medpor artificial sphere graft, and covering the anterior surface of the sphere with an autologous dermis-fat graft.
Results
The Medpor graft was successfully placed on the inferior orbital rim with non-absorbable sutures after preparing the orbital floor and retrieving tissues from the ipsilateral maxillary sinus. The hydroxyapatite-Medpor artificial sphere was placed after removing scar tissue, and its anterior surface was covered with an autologous dermis-fat graft taken from the abdominal area. Simultaneously, eyelid fornices were created on the anterior surface of the graft by placing an orbital conformer and suturing the eyelids together.
Conclusion
Satisfactory orbital fullness was achieved with moderate enophthalmos and an inability to close the eyelids (lagophthalmos).
