Müller’s muscle conjunctival resection surgery with an external approach in Horner Syndrome

Purpose

Demonstrate a case of Müller’s muscle conjunctival resection surgery performed with an external approach in Horner Syndrome.


Methods

42-year-old male with a history of surgical excison of a cyst near the left lung 2 months ago, presented to Oculoplastic departament with mild ptosis of the left eye since the surgery. Examination showed good levator function, positive response to 10% phenylephrine drop and a miotic left pupil that dilates poorly in response to darkness. Due to the sympathetically innervation of Müller’s muscle and the relation with Horner syndrome, Müller’s muscle conjunctival resection with an internal approach is often the surgery of choice for ptosis correction in patients with this syndrome. The present case shows the Müller’s muscle conjunctival resection performed with an external approach in order to avoid corneal and fornix injuries.


Results

Müller’s muscle conjunctival resection with an external approach effectively achieves postoperative symmetry success with great cosmetic outcome during the follow-up of 4 months. No complications were noted like tear film instability, keratopathy or eyelid contour abnormality.


Conclusion

Although Müller’s muscle conjunctival resection is a well-established surgical technique used in patients with mild ptosis and good levator function, we showed the external approach of this surgery that can be an alternative technique in selected cases. In addition it can be useful in cases that require skin removal because we prevent the necessity of external and internal approach together.
