TWO CASES OF NONTRAUMATIC ORBITAL SUBPERIOSTAL HEMATOMA

Purpose
Nontraumatic orbital subperiosteal hematoma is uncommon. In this case study we aim to illustrate the heterogeneity of cause, presentation and need for surgical intervention.
Methods
We present two cases of subperiosteal hematoma of the orbit that are not caused by head trauma.
Results
In the first case, a 32-years-old woman presented to our clinic one day postpartum with normal visual acuity, unilateral periorbital swelling, proptosis, ptosis and diplopia due to mild pupil-sparing third-nerve palsy. CT orbit revealed a subperiosteal hematoma with proptosis. There was improvement of symptoms within a day and a complete resolution of symptoms within four weeks. The second case involved a 23-years-old woman who presented one day after a strangulation suicide attempt. She had unilateral periorbital hematoma, proptosis, ptosis, painful eye movements and decrease in visual acuity. CT orbit showed a subperiosteal hematoma and stretching of the optic nerve. This patient underwent a surgical drainage, which resolved the orbital symptoms.
Conclusion
Nontraumatic orbital subperiosteal hematoma can have various etiology, presentation and disease courses. Recognition of vision-threatening signs is imperative, emphasizing the importance of early surgical drainage to achieve favorable outcomes.
