Brow Volume Augmentation with Lower Eyelid Fat Pearls

Purpose
Upper eyelid fat transposition and autologous fat transfer (AFT) are routinely used at the time of upper blepharoplasty to correct brow deflation. However, fat transposition is limited by the amount of available fat and AFT has variable results. We are proposing a new technique of brow volume augmentation using fat pearls harvested from the lower eyelid at the time of blepharoplasty.
Methods
Video presentation of surgical technique and retrospective chart review of the last 20 consecutive patients treated with brow fat pearls. A blind observer was asked to grade the improvement in brow convexity and fulness in each operated eye using a scale from 0 to 5 where 0 was given for no improvement and 5 for fully convex brow.
Surgical technique: A pocket was created pre-periosteally (inside the ROOF) above the superior orbital rim under the lateral half of the brow measuring 15 x 25 mm. The non-transposable excess lower eyelid fat was trimmed into 5 mm diameter pearls and transferred into the ROOF pocket till the pocket was completely full. The fat was secured in place with horizontal mattress 4.0 PDS sutures anchoring the superficial galea to the periosteum.
Results
All patients included in the study were very satisfied with the final cosmetic result. There were no major complications seen in either group. The improvement in brow convexity was 4.27+/-0.07 in the study group and2.52+/-0.09 in the control (Blepharoplasty only) group (p<0.01).
Conclusion
Brow volume augmentation with lower eyelid fat pearls is a safe and effective procedure. The degree of brow volume augmentation is directly proportional to the amount of fat pearls transferred, hence dependent on the lower eyelid fat excess. Although, in our patients, most of this fat came from the lateral fat pocket, in certain cases central or medial excess, un-transposable fat was also used.
Although a larger, prospective study is certainly warranted, this small retrospective study supports the safety and efficacy of brow volume augmentation using lower eyelid fat pearls at the time of upper and lower blepharoplasty.
