Punctal stenosis in relation to dry eyes.

Purpose
To describe the prevalence of punctal stenosis in the setting of dry eyes and its implications on the management of dry eye symptoms.
Methods
This was a retrospective study about the prevalence of punctal stenosis in patients who suffers from dry eyes. controlled group was made of patients who were coming for refractive surgery consultation without signs or symptoms of dry eyes. Age range was from 20 to 50 year old. included patient had healthy eyelid margins, or only grade one posterior blepharitis, grade 1 Demodex Follicularum (DF) infestation. Punctal shape was the circular type. Exclusion criteria, were posterior blepharitis grade 2 or more, DF infestation grade 2 or more, slite-shaped punctum. Dry eyes were graded as mild, moderate, severe or none. Punctum was graded as open, stenosed or obstructed. Mann-Whitney test was used to compare the prevalence of punctal stenosis in both groups.
Results
There was 31 patient with dry eyes and 29 patient in the controlled group. there was significant difference in the prevalence of punctal stenosis or obstruction in patients with dry eyes in comparison to normal patients (92% vs 20% P=0.02%)
Conclusion
the majority of Patients with dry eyes, already have punctal stenosis or obstruction, which represent the natural development of punctal size, in the face of dry eye. The changes in punctal size help the eye to delay the symptoms and signs of dry eye. These findings would affect the management of dry eyes, in particular the methods of blocking the punctum, as most of these patients will already have punctal stenosis. Punctal size assessment should be included in the dry eye management and the prognosis of dry eyes development after laser refractive surgery.
