Using Skin Optical Coherence Tomography to Distinguish Hidrocystoma from Basal Cell Carcinoma.

Purpose

To evaluate the effectiveness of Skin Optical Coherence Tomography (OCT) in differentiating benign hidrocystomas from malignant cystic nodular basal cell carcinomas (BCC) in the periocular region, with the aim of reducing unnecessary biopsies and improving early cancer detection.


Methods

A prospective clinical study at the University Hospital of Southern Denmark including patients with suspected periocular BCC. A commercial 1310 nm OCT system with a handheld probe is used. Prior to skin biopsy, patients undergo OCT scanning of the skin lesion. Scanning results are then compared with histological findings. To date, 92 of the planned 210 patients have been included, with biopsy results available for 86. Early observations suggest OCT of periocular skin effectively differentiates hidrocystomas from cystic nodular BCCs.


Results

Among the 86 evaluated lesions, a mixed cystic and nodular BCC was found in three cases, and a benign hidrocystoma was found in five cases. OCT findings revealed that hidrocystomas had well-defined cystic margins with normal overlying skin, whereas cystic nodular BCCs appeared poorly defined with disrupted skin layers. In one case, diagnostic uncertainty led to a consideration of incision and demonstrated the role of Skin OCT in reducing diagnostic uncertainty, highlighting its potential clinical value. Scanning images further illustrate these distinct morphological differences.


Conclusion

Skin OCT is a promising, fast non-invasive bedside tool for periocular lesion assessment. A newly developed handheld probe enhances scanning accuracy in curved areas. Our findings suggest that Skin OCT improves differentiation of hidrocystomas from cystic nodular BCCs, aiding precise treatment planning and surgical decision-making in this sensitive region.
