Lower eyelids blepharoplasty: it’s all about the fats

Purpose

Lower eyelids blepharoplasty is one of the most common cosmetic eyelid procedures. Through lower eyelids we could manage manage different abnormalities in the lower eyelid, malar & mid face areas. The fat in this area is usually of great benefit when dealt with it appropriately. Preserving fat along the transconjunctival approach is a useful technique with minimal complications.


Methods

A transconjunctival blepharoplasty is described in which an adequate amount of fat is preserved and left to maintain an adequate volume preventing hollowness and providing a fresh & rejuvenated appearance. This has been the technique of choice performed in more than 250 eyelids. All cases were done under local anaesthesia. The conjunctiva is left sutureless.


Results

All cases had a subjective improvement of eyelid puffiness with a natural volume. The occurrence of hollowness is rare. Less than 4% of the cases had concerns with volume or skin redundancy.


Conclusion

Transconjunctival blepharoplasty with fat preservation is a minimally invasive procedure with a high satisfactory level among cases. Along the rapid & smooth downtime it provides a natural appearance desired by the patients.
