Predicting Long-Term Outcomes After MMCR: The Role of Immediate Postoperative Assessment

Purpose
Postoperative assessment of ptosis surgery outcomes integrates objective eyelid position and surgeon satisfaction. Edema, hemorrhage, and scarring can complicate early predictions. This study investigated how early postoperative evaluations correlate with final outcomes after Muller’s Muscle-Conjunctival Resection (MMCR).
Methods
A retrospective cohort analysis was conducted on MMCR patients. Surgeon satisfaction was graded ordinally and binarily. MRD1 (Margin Reflex Distance 1) measurements were recorded preoperatively, immediately postoperatively, at one-week (POW1), and final follow-up. Spearman’s correlation assessed relationships between satisfaction and MRD1; logistic regression analyzed if early dissatisfaction predicted final dissatisfaction.
Results
Thirty-nine patients (mean age 57.5 years) were included. MRD1 showed steady improvement over time. Immediate MRD1 moderately correlated with POW1 MRD1 (r = 0.58, p = 0.01) and final MRD1 (r = 0.45, p = 0.1), while POW1 MRD1 strongly correlated with final MRD1 (r = 0.904, p < 0.001). Strong correlations were found between MRD1 and surgeon satisfaction at all timepoints (p < 0.05). Immediate and POW1 satisfaction scores were moderately correlated (r = 0.35, p = 0.14). Logistic regression showed immediate dissatisfaction did not significantly predict final dissatisfaction (OR 1.18, p = 0.77). Immediate and final satisfaction matched in 50% of cases.
Conclusion
Immediate postoperative assessments were not predictive of final outcomes. However, evaluations at one week postoperatively provided prognostic value, and MRD measurements taken at one week were highly predictive of final eyelid position outcomes. These findings support the clinical importance of early postoperative follow-up and objective MRD1 monitoring to optimize outcome prediction.
