Restoring sight and smile: Combined corneal neurotization with facial reanimation surgery

Purpose

To study the efficacy of combined corneal neurotization and facial nerve reanimation surgery in patients with multiple cranial nerve palsies


Methods

This study included 8 patients who developed neurotrophic keratitis and exposure keratopathy due to fifth and seventh nerve palsy following neurosurgical procedures. All patients underwent a combined indirect corneal neurotization and facial reanimation surgery. The corneal sensations, ocular surface and visual acuity were assessed at the end of 3 months, 6 months and 1 year


Results

All patients showed an improvement in corneal sensations and ocular surface after 6 months.Visual acuity improved in all but one patient who had developed corneal scarring preoperatively.


Conclusion

Corneal neurotization is a proven technique for improving corneal sensations in neurotrophic keratitis. However correction of a co-existing lagophthalmos improves visual outcomes in patients with a combined etiology of keratitis.
