Comparison of Electrocautery and Scalpel Incision for Upper Eyelid Blepharoplasty

Purpose
Electrocautery provides a hemostatic benefit for skin incision; however, its effect on scar cosmesis remains unclear. We aimed to compare the Colorado needle electrocautery pure cutting mode with the traditional scalpel to determine their efficacy, complications, and cosmetic outcomes.
Methods
A prospective, intraindividual, randomized, controlled trial was conducted to compare the effectiveness of Colorado needle electrocautery and scalpel in upper blepharoplasty. Study outcomes included scar quality, incision time, incision bleeding, postoperative edema, ecchymosis, and aesthetic outcome at various times up to 6 months postoperatively.
Results
The prospective randomized controlled trial included 82 eyelids of 41 patients; the mean incision time was significantly shorter on the electrocautery side (group 1) than on the scalpel side (group 2), and there was less blood loss during incision on the electrocautery side than on the scalpel side (P < 0.05). Postoperative day 1 and week 1 ecchymosis was significantly less on the electrocautery side than on the scalpel side (P < 0.05). The two groups had no significant difference in postoperative edema and aesthetic outcome. Scarring was significantly more pronounced in the electrocautery group at 1 month, 3 months, and 6 months postoperatively (all P < 0.05). Hypopigmented scarring was more common on the scalpel side and hyperpigmented scarring was more common on the electrocautery side; however, this difference was not statistically significant.
Conclusion
The use of electrocautery has a hemostatic advantage, resulting in less bleeding at the incision site, which may be the reason why ecchymosis is less common in the postoperative period in this group. However, the incision time on the electrocautery side is significantly shorter than on the scalpel side. Scalpel incisions are less prominent than Colorado needle incisions in terms of scarring after upper lid blepharoplasty, but there was no significant difference in aesthetic appearance.
