A Comparison of Fibrin Glue and Nasal Packing for Hemostasis After Endonasal Dacryocystorhinostomy: A Retrospective Study

Purpose

To compare the effectiveness of fibrin glue versus nasal packing in preventing clinically significant nasal bleeding requiring intervention after endonasal dacryocystorhinostomy (en-DCR), and to assess whether fibrin glue affects surgical outcomes or complication rates.


Methods

A retrospective analysis was performed on 54 patients (69 eyes) who underwent primary en-DCR for nasolacrimal duct obstruction between January 2022 and December 2023. Group A included 24 patients (30 eyes) who received nasal packing postoperatively, while Group B included 30 patients (39 eyes) who were treated with the intraoperative application of fibrin glue at the osteotomy site without packing. All surgeries were conducted under general anesthesia on a day-surgery basis. The primary endpoints were nasal bleeding requiring hemostatic intervention, anatomical success (defined by patent rhinostomy on irrigation), and functional success (defined by complete resolution of epiphora).


Results

No patients in either group experienced postoperative nasal bleeding that required medical or surgical hemostasis (0/30 in Group A, 0/39 in Group B). The mean operative time was 42.3 ± 6.1 minutes in Group A and 43.1 ± 5.9 minutes in Group B (p=0.52). No intraoperative or postoperative complications such as orbital trauma, infection, or granuloma formation were observed in either group. The anatomical and functional success rates were 96.7% (29/30) in the nasal packing group and 100% (39/39) in the fibrin glue group, indicating excellent outcomes in both cohorts.


Conclusion

Fibrin glue was as effective as nasal packing in preventing significant postoperative nasal bleeding in en-DCR. It demonstrated comparable safety, success rates, and operative time, and also avoided the discomfort associated with nasal packing. These findings support the use of fibrin glue as a favorable alternative for routine en-DCR procedures.
