Outcomes of Simultaneous Orbital Fat Decompression and Autologous Fat Grafting to the Midface in Patients with Thyroid Eye Disease

Purpose

In thyroid eye disease (TED), inflammation and hypertrophy of orbital tissue result in proptosis, often accompanied by lower eyelid retraction and shadowing under the eyes. We routinely perform orbital fat decompression for cosmetic improvement and have adopted a technique using excised orbital fat for autologous grafting to the lower eyelid and midface. This report presents our outcomes with this approach.


Methods

Sixteen patients (32 eyes) who underwent orbital fat decompression and simultaneous fat grafting to the lower eyelid for aesthetic purposes in 2023 were included. Under general anesthesia, orbital fat was removed via a transconjunctival approach from both intraconal and extraconal spaces. Approximately 0.5 cc of excised orbital fat was grafted under the Orbicularis Retaining Ligament (ORL) into the lower eyelid and midface. Postoperative outcomes and complications were retrospectively reviewed.


Results

The mean patient age was 45.3 ± 12.8 years; 14 were female. The average volume of excised fat was 3.3 ± 0.9 cc. Preoperative proptosis measured 22.2 mm on average, which improved to 16.5 mm postoperatively. The mean MRD2 decreased from 7.28 ± 1.09 to 6.17 ± 0.51 mm. In all cases, the infraorbital hollowing and “dark circles” resolved, and no visual impairment or major complications occurred.


Conclusion

Orbital fat decompression combined with autologous fat grafting offers a comprehensive solution for the disfiguring features of TED, effectively addressing both functional and aesthetic concerns. This technique is safe, with minimal complications, and provides significant cosmetic improvement.
