Purpose
Evaluate surgical outcomes and oncologic control in malignant eyelid tumors treated with systematic frozen section–controlled excision and reconstruction, with specific focus on lacrimal drainage apparatus involvement.
Methods
Retrospective case series of 41 consecutive patients with histopathologically confirmed malignant eyelid tumors treated January 2020–December 2023. Study adheres to STROBE guidelines and received IRB approval. All tumors underwent systematic intraoperative frozen section margin control with targeted re-excision of positive margins: initial negative margins 35/41 (85%), positive margins 6/41 (15%), final negative margins 41/41 (100%). Reconstruction employed local flaps, tarsoconjunctival grafts, or full-thickness skin grafts. Primary outcomes: recurrence rate and lacrimal involvement. Categorical variables compared using chi-square test. Mean follow-up 14.5 months (median 14, IQR 13–16), range 12–23 months, 100% completion.
Results
Cohort: 21 males, 20 females; mean age 64 years. Distribution: lower 27 (66%), upper 7 (17%), medial canthal 7 (17%). Histology: basal cell 33 (80%), squamous 4 (10%), basosquamous 2 (5%), adenoid cystic 1, Merkel 1. Local recurrence: 3 patients (7.3%, CI 1.6–19.5%)—2 basal cell, 1 squamous—all successfully re-excised. Recurrence timing: mean 8.3 months. Medial canthal tumors with lacrimal involvement (n=7): canalicular excision and sac ablation performed; epiphora 100% (mild 3, moderate 3, severe 1); recurrence 14.3% (1/7). Cicatricial ectropion 2 (4.9%), surgically corrected. Ocular surface disease in 43% of epiphora cohort. No graft failure or metastases.
Conclusion
Frozen section margin control with 100% final negative margins achieves low recurrence (7.3%) and acceptable morbidity. Lacrimal involvement requires aggressive surgery resulting in inevitable epiphora (100%) and higher recurrence (14.3%). Systematic margin control with tailored reconstruction is essential in eyelid tumor management.
