Ectopic Lacrimal Gland Pleomorphic Adenoma Presenting as an Upper Eyelid Mass: A Case Report

Purpose
To report a rare case of pleomorphic adenoma arising from ectopic lacrimal gland tissue presenting as an upper eyelid mass, and to highlight previously reported cases of pleomorphic adenoma arising from ectopic lacrimal gland tissue in the literature.
Methods
Case report of a 14-year-old girl with a painless, progressively enlarging right upper eyelid mass evaluated by clinical examination and orbital MRI. Definitive management was done by complete surgical excision via an anterior orbitotomy with histopathologic confirmation.
Results
Clinical exam showed a localized, non-tender nodular lesion without ptosis or proptosis. MRI demonstrated a well-defined, homogeneously enhancing extraconal lesion confined to the lateral upper eyelid. The lesion was completely excised without complications. Histopathology demonstrated a well-circumscribed, encapsulated tumor composed of myoepithelial cells within a chondromyxoid stroma, with adjacent normal lacrimal gland tissue, consistent with pleomorphic adenoma arising from an ectopic lacrimal gland tissue. Postoperative follow-up showed satisfactory healing with no evidence of recurrence.
Conclusion
Pleomorphic adenoma arising from ectopic lacrimal gland tissue is exceedingly rare and may pose a diagnostic challenge due to its atypical location. Despite the very few cases reported in the literature, careful clinical evaluation, appropriate imaging, and definitive histopathologic assessment following complete surgical excision are essential to establish diagnostic certainty and achieve optimal outcomes. Review of previously reported cases demonstrates that pleomorphic adenoma arising from ectopic lacrimal gland tissue has been described in various periocular and orbital locations, with only a single prior case reported in the upper eyelid. This case therefore represents the second reported upper eyelid case and the first reported pediatric case in this anatomical location.
