Natural History and Long-Term Outcomes of Optic Disc Melanocytoma in 225 Cases

Purpose

To describe the natural history of optic disc melanocytoma (ONM), a rare, benign pigmented tumour which can cause visual morbidity and rarely, malignant transformation.


Methods

This retrospective observational case series included 225 patients diagnosed with ONM between November 1999 and August 2023. Analysis included demographics, tumour location, tumour characteristics, imaging findings, and outcomes including tumour growth, visual field defects, optic nerve complications, and malignant transformation.


Results

Among 225 patients (64.9% female; mean age 52.9 years), 75% were followed up for an average of 6.8 years. ONM was confined to the optic disc in 23.1%, adjacent to the optic disc in 10.2%, and involved both in 61.3% of patients. Visual field defects were observed in 43.9% of the patients, retinal nerve fibre layer (RNFL) thinning in 23.3%, tumour growth in 6.6%, and malignant transformation in 1%.


Conclusion

ONM is predominantly benign but carries risks of visual morbidity and rare malignant transformation.
