Multiple bilateral periocular hidrocystomas associated with hyperprolactinaemia: a multicentre case series

Purpose
Multiple bilateral periocular hidrocystomas is rare. We present a case series of such patients with concurrent hyperprolactinaemia. The role of prolactin in possible causation is explored.
Methods
A retrospective observational case series was conducted across three UK oculoplastic units. Patients presenting between January 2010 and May 2024 with multiple (>2), bilateral periocular hidrocystomas and documented serum prolactin testing were identified. Demographic, clinical, biochemical, radiological, histological, treatment, and outcome data were collected. Patients were grouped by aetiology of hyperprolactinaemia (prolactinoma vs drug-induced). Prolactin levels were compared using a Welch’s t-test.
Results
Seven patients were included (mean age 56 years, range 41–78; five males). Mean serum prolactin level was 11,375 mIU/L (range 1,075–42,373). Four patients had prolactinomas on MRI, while three had drug-induced hyperprolactinaemia. Prolactin levels were significantly higher in the prolactinoma group than in the drug-induced group (18,796 vs 1,480 mIU/L; p = 0.0002). All patients with prolactinomas responded to cabergoline therapy.
Conclusion
This series highlights an association between multiple bilateral periocular hidrocystomas and hyperprolactinaemia. Although causality cannot be established, clinicians encountering multiple bilateral or recurrent periocular hidrocystomas should consider serum prolactin testing to avoid missing significant underlying endocrine disease. It is also important to consider that hidrocystoma excision is subject to funding restrictions which may result in patients not being referred and a missed opportunity to diagnose these patients early.
