Endoscopic Approach Temporal Brow Lift With Novel Vector For Optimal Enhancement To Brighten Upper Eyelid Blepharoplasty Results

Purpose
To evaluate the effectiveness and safety of combining an endoscopic temporal brow lift with upper eyelid blepharoplasty to optimize eyelid crease aesthetics and reduce postoperative brow descent, thereby improving overall periorbital brightness and patient satisfaction.
Methods
From the point medial to the temporal line of fusion, in-line with the brow peak, a 2-cm curvilinear posterior hairline scalp incision is carried laterally to allow for the optimal vector superiorly rather than temporally. A standard subperiosteal dissection is carried medially through the temporal fusion line to the medial head of the brow and inferiorly releasing the temporal brow periosteal attachments. The brow vector of lift is maximal in this location with the optimal tissue glide plane advancing and secured to a monocortical bone channel with 4-0 monocryl suture. This is contrasted to other commonly utilized fixation techniques. An upper eyelid blepharoplasty is performed in an individualized fashion to achieve a natural optimal contour of the upper lid fold displaying an appropriate eyelid platform throughout.
Results
The endoscopically assisted temporal brow lift technique is designed to achieve tissue release under direct visualization. The brow lift vector of pull described is centered over the brow peak while adequately lifting the brow tail to improve lateral upper eyelid fold height and a smooth contour of the pretarsal space.
Conclusion
Combining endoscopic style temporal brow lift and the described unique vector of lift with upper eyelid blepharoplasty provides durable lateral brow elevation that complements eyelid skin resection, improving eyelid aperture, lateral crease exposure, and overall periorbital brightness. The described minimally invasive endoscopic approach reduces visible scarring while allowing precise vector control. Careful pre operative evaluation and patient selection with blepharoplasty planning are essential to maximize functional and aesthetic outcomes.
