The Color Ring Test as an Alternative to Preoperative Conventional Perimetry for Ptosis and Dermatochalasis

Purpose
To evaluate The Color Ring Test as a rapid, cost-effective alternative to conventional visual field testing (Goldmann Field Test [GFT] and Humphrey Field Test [HFT]) for preoperative assessment of superior visual field loss in patients with ptosis and dermatochalasis.
Methods
Prospective cohort study conducted in Detroit, USA and Melbourne, Australia (March 2023–December 2024). Consecutive adult patients undergoing evaluation for functional upper eyelid surgery were included. Each eye was tested with the Color Ring Test followed by GFT (Detroit) or HFT (Melbourne). Testing was performed with the eyelid untaped and taped. Surgical eligibility was defined as ≥12° or ≥30% improvement in superior visual field after taping. Concordance, sensitivity, and specificity of the Color Ring Test were calculated against local standard perimetry
Results
At the Detroit center, 211 eyes met surgical criteria by GFT; 167 eyes (79.1%) also met criteria using the Color Ring Test. In Melbourne, 54 eyes met criteria by HFT; 42 eyes (77.8%) also met criteria by the Color Ring Test. Combined analysis demonstrated overall sensitivity of 0.879 (95% CI 0.834–0.915) and specificity of 0.779 (95% CI 0.726–0.826). Bilateral testing required approximately 4–5 minutes compared to 30-minute scheduling for standard perimetry.
Conclusion
The Color Ring Test demonstrates good concordance with conventional perimetry and high sensitivity for detecting superior visual field loss due to upper eyelid malposition. It represents a rapid and potentially cost-effective screening alternative for functional ptosis and dermatochalasis assessment.
