Endoscopic assisted lacrimal reintubation versus external dacryocystorhinostomy in persistent congenital nasolacrimal duct obstruction, a comparative study

Purpose

To compare the success rate of bicanalicular silicone re-intubation using nasal endoscopy versus external dacryocystorhinostomy (ex-DCR) for the treatment of persistent congenital nasolacrimal duct obstruction after previous failed conventional intubation


Methods

40 eyes of 40 patients with CNLDO ,who underwent previous failed conventional intubation were divided into two groups 20 eyes undewent endoscopic silicone intubation (group A) , and 20 eyes underewent ex-DCR (group B) with silicone intubation. Postoperative Munk score, tear film height, florescine disappearance test (FDDT) and parent satisfaction were compared between the two groups.


Results

Functional and anatomical success (Munk score, FDDT and tear film height) were comparable in both procedures 85 % in group (A) & 80 % in group (B). Endoscopic reintubation surgery was found to have shorter surgical duration, less risk of blood loss, faster rehabilitation and more parents’ acceptance to perform than external DCR surgery with no risk of scar formation


Conclusion

Endoscopic reintubation surgery offers a good alternative to the well established technique of ex-DCR surgery for the treatment of persistant congenital nasolacrimal duct obstruction with comparable success rates, shorter surgical time and higher patient satisfaction.
