Single-Stage Surgical Management of Cicatricial Ectropion Secondary to Rosacea Using Malar Lift and Tripier Flap

Purpose
To describe the surgical management of cicatricial ectropion secondary to rosacea using a combined approach with malar lift and Tripier flap in a single-stage procedure, and to evaluate its functional and aesthetic outcomes.
Methods
A 77-year-old male with a long-standing history of rosacea presented with epiphora, chronic ocular irritation, and corneal exposure. On examination, cicatricial ectropion of the lower eyelid was observed, with skin retraction, eversion of the eyelid margin, and incomplete eyelid closure, associated with cutaneous changes consistent with rosacea.
Results
Given the significant cicatricial component and vertical shortening, surgical treatment was performed using a malar lift to reposition the midface and release inferior traction, combined with anterior lamellar reconstruction using a Tripier flap.
The procedure was carried out in a single surgical stage, achieving proper repositioning of the lower eyelid, improved eyelid closure, and resolution of epiphora. No significant postoperative complications were observed, with good functional and aesthetic outcomes.
Conclusion
DISCUSSION
Cicatricial ectropion associated with rosacea represents a surgical challenge due to chronic inflammation, skin contracture, and altered tissue quality. Management should address not only the eyelid malposition but also the underlying midface support.
The malar lift reduces vertical traction on the lower eyelid, improving its position and decreasing recurrence rates. The Tripier flap provides well-vascularized tissue for anterior lamellar reconstruction and is particularly useful in cases with skin deficiency.
The combination of both techniques in a single-stage procedure may yield satisfactory results, avoiding staged surgeries and optimizing patient recovery.
CONCLUSIONS
The combined approach of malar lift and Tripier flap is an effective option for the management of complex cicatricial ectropion, particularly in patients with rosacea. It restores eyelid anatomy, improves function, and achieves favorable aesthetic outcomes with a low complication rate.
