Botulinum Toxin Therapy as a Minimally Invasive Option for Bilateral Lower Eyelid Entropion in an Anticoagulated Patient

Purpose
To evaluate the effectiveness of botulinum toxin injections as a minimally invasive treatment for bilateral lower eyelid entropion in a patient receiving anticoagulant therapy.
Methods
A patient with bilateral involutional lower eyelid entropion, receiving anticoagulant therapy, was treated with botulinum toxin type A injections. A total dose of 10 units per eyelid was administered into the pretarsal orbicularis oculi muscle, distributed as 2 units per injection point. The procedure was performed under aseptic conditions without discontinuation of anticoagulant therapy.
Results
Clinical improvement was observed starting from the first day after injection, with full correction of eyelid position achieved within 14 days. The therapeutic effect was stable and lasted up to 6 months. No complications, including hemorrhagic events, were noted.
Conclusion
Botulinum toxin injection into the pretarsal orbicularis muscle is a safe and effective minimally invasive option for managing lower eyelid entropion, particularly in patients with increased surgical risk due to anticoagulant therapy.
