Reconstruction of Lateral Canthus Defects Following Eyelid Tumor Excision: A Case Series

Purpose
The lateral canthus represents a complex anatomical and functional unit, and its reconstruction following tumor excision poses significant surgical challenges. Achieving both functional stability and satisfactory cosmetic outcomes requires careful selection of reconstructive techniques.
Methods
This case series included five patients with periocular tumors involving the lateral canthus who underwent complete surgical excision followed by reconstruction. Reconstructive techniques were selected according to defect size and depth, including the Tripier flap and periosteal flap. All patients were followed for a minimum of 6 months. Functional outcomes (eyelid position, canthal stability) and aesthetic results were assessed.
Results
All tumors were successfully excised with histologically clear margins. Reconstruction resulted in satisfactory functional and cosmetic outcomes in all patients. The periosteal flap provided excellent lateral canthal support and stability, while the Tripier flap ensured adequate soft tissue coverage with good aesthetic integration. Minor postoperative complications, including transient edema and mild asymmetry, were observed, without the need for revision surgery. No tumor recurrence was detected during follow-up.
Conclusion
Reconstruction of lateral canthus defects can be effectively achieved using tailored surgical approaches. The periosteal flap offers superior structural support, while the Tripier flap is reliable for soft tissue reconstruction. Individualized technique selection is key to optimizing both functional and aesthetic outcomes.
