Digital Decision Support for Structured Thyroid Eye Disease Assessment: A Pilot Feasibility Study in Ophthalmology Residents

Purpose
Thyroid eye disease assessment requires integration of activity scoring, severity stratification, recognition of sight-threatening features, and structured documentation. In busy outpatient clinics, this process may be difficult for ophthalmology residents to standardize. This pilot study evaluated an artificial intelligence-assisted application designed to support structured thyroid eye disease assessment based on the principles of the European Group on Graves’ Orbitopathy (EUGOGO).
Methods
A mobile and web-based application was developed to support structured thyroid eye disease assessment. The application included EUGOGO-based clinical activity scoring, severity classification, identification of sight-threatening features, a structured ophthalmic examination form, Hertel entry, image upload, archive review, and guideline-based recommendations.Twenty-one ophthalmology residents evaluated the application and completed a questionnaire addressing usability, workflow clarity, decision support, educational value, documentation utility, and telemedicine potential.
Results
Twenty-one residents completed the evaluation. Clinical decision support was endorsed by all participants (100%). Overall usefulness was rated positively by 95.2% of respondents (mean 4.90/5). The interface was considered clear by 95.2% (mean 4.71/5), and 90.5% found data entry practical (mean 4.57/5). Classification was considered logical by 95.2% (mean 4.71/5), and 95.2% rated the recommendations as guideline-consistent (mean 4.86/5). Educational value was rated positively by 95.2%, routine clinical usability by 71.4%, and telemedicine potential by 85.7%.
Conclusion
This digital application appears feasible as a structured decision-support and documentation tool for ophthalmology residents and may also be valuable for early-career ophthalmologists assessing thyroid eye disease. It may improve consistency, reduce uncertainty, and support documentation and education in busy clinical settings.
