Surgical Management of a High-Flow Lower Eyelid Arteriovenous Malformation After Complicated Embolization: 5-Year Outcomes

Purpose
To report the management and long-term outcome of a rare high-flow lower eyelid arteriovenous malformation following complicated embolization.
Methods
A 40-year-old male presented with a 19-year history of a progressive left lower eyelid mass. Examination revealed a bluish pulsatile lesion with palpable thrill. Imaging confirmed a high-flow arteriovenous malformation. An embolization attempt was complicated by status epilepticus requiring intensive care, leading to abandonment of further endovascular treatment.
Results
Complete surgical excision was performed, requiring total lower eyelid resection. Reconstruction was achieved using a staged approach combining an oblique frontal flap and fibromucosal graft. Secondary revision and flap division were performed. At 5-year follow-up, the patient showed stable eyelid reconstruction with preserved ocular protection, no recurrence, and satisfactory aesthetic outcome.
Conclusion
In periocular arteriovenous malformations, surgical excision remains a reliable option when embolization is contraindicated or complicated. This case demonstrates the durability of staged reconstruction with favorable long-term functional and aesthetic results.
