Purpose

To report a case of a congenital complex choristoma involving the orbit and the eyelid

Methods

We report a case of a female newborn presenting a large lesion affecting the nasal half of the right upper eyelid. Magnetic resonance imaging of the orbits demonstrated an exophytic lesion, extending from the right upper eyelid into the right superior orbit and fully repressed by fat suppression sequence.
This lesion was located extraconally, and hairs were observed on its eyelid-involved portion. All other ocular structures appeared normal.

Results

At three months of age, the lesion was completely excised and submitted for histopathological evaluation, followed by reconstruction of the right upper eyelid.
Gross examination of the tissue showed a lobulated yellow mass. Histological inspection of it revealed lobules of mature adipose tissue and small islets of lacrimal gland tissue, as well as a tiny nodule of cartilage. These findings were consistent with the diagnosis of a complex choristoma.

Conclusion

Ocular choristomas are rare lesions that may involve the conjunctiva, eyelids, orbits or a combination of ocular tissues. There is only a limited number of reports regarding complex choristoma of the orbit and eyelid with no associated diagnosed syndrome. This case adds to the existing literature by describing another instance of combined orbital and eyelid complex choristoma in a newborn without known systemic disease.
