Impact of Graves’ Disease Aggravation and Recurrence on the Clinical Course of Thyroid-Associated Orbitopathy

Purpose
To evaluate longitudinal changes in ophthalmic findings following aggravation or recurrence of Graves’ disease (GD) in patients with thyroid-associated orbitopathy (TAO), and to identify factors associated with these changes.
Methods
This retrospective study included 138 patients with TAO (150 episodes) who experienced aggravation or recurrence of GD. Episodes were classified into aggravation and recurrence groups based on predefined criteria reflecting prior treatment status and disease course. Demographic characteristics and ophthalmic findings at baseline, onset, and at 3 and 6 months were analyzed. Changes in ophthalmic findings was assessed by comparing each time point with both baseline and onset values.
Results
In the aggravation group, the clinical activity score (CAS) increased at onset compared with baseline (P = 0.002) and subsequently decreased, while proptosis and margin reflex distance 1 (MRD1) remained stable. In contrast, the recurrence group showed no significant change in CAS or proptosis, whereas MRD1 increased and remained elevated (all P ≤ 0.001).
Conclusion
Aggravation and recurrence of GD differentially affect ophthalmic findings in TAO. Aggravation is primarily associated with increased inflammatory activity, whereas recurrence is more closely associated with eyelid retraction.
