Surgical burden and success of oculoplastic procedures in ocular cicatricial pemphigoid

Purpose

To investigate the long-term surgical burden and clinical success of oculoplastic interventions in patients with ocular cicatricial pemphigoid (OCP).


Methods

This retrospective single center study analyzed 28 patients (56 eyes) with OCP requiring at least one eyelid procedure. The median age at diagnosis was 68 years with a median follow-up of 7.4 years (range: 2-24). Evaluation focused on surgical frequency per eye, time-dependent complication rates, and the efficacy of anterior lamellar repositioning (ALR).


Results

Structural eyelid pathologies showed high prevalence: trichiasis (96%) and entropion (86%). They occurred early after diagnosis: the median time to onset was 3.5 months for entropion and 1.8 months for trichiasis. The median surgical load was 2 interventions per eye (range 0–19). ALR combined with mucous membrane grafting (MMG) achieved high stability, with a 5-year entropion-free survival rate of 92.9% for upper lids. Without MMG, 32% of lids required surgical re-intervention. Electroepilation showed a median recurrence-free survival of 1.5 years (5-year rate: 30.7%). Anatomical assessment showed that the ocular surface remained stable in 50% of eyes, while 21% showed improvement following surgery.


Conclusion

OCP is characterized by a high surgical burden and requires surgical eyelid procedures to avoid further damage to the ocular surface. Structural eyelid pathologies occurred early after OCP diagnosis. ALR with MMG is an effective causal therapy for anatomical stabilization, whereas symptomatic treatments like electroepilation offer limited long-term success.
