Temporal brow lift case report

Purpose
This case report aims to evaluate the outcomes and complications associated with the temporal brow lift procedure, particularly focusing on nerve damage and aesthetic results. The procedure is intended to rejuvenate the upper face by addressing sagging eyebrows, forehead wrinkles, and facial asymmetry.
Methods
A 73-year-old female patient presented with complaints of heaviness in her eyebrows, predominantly on the left side. Under general anesthesia, a temporal brow lift was performed without endoscopic assistance. Intraoperative observations were made regarding the technical aspects of the procedure, including incision placement, brow fixation, and management of complications. Postoperative assessments included monitoring for facial asymmetry and nerve function over a five-month recovery period.
Results
The procedure successfully elevated the right eyebrow without complications. However, during the left eyebrow lift, trauma to the Seinsel vein resulted in a subcutaneous hematoma and subsequent asymmetry. Post-surgery, the patient exhibited swelling, left eyebrow ptosis, and temporary unilateral dysfunction of the frontal muscle, indicating neurapraxia of the left temporal branch. Over five months, the patient regained full function of the facial nerve, achieving improved brow positioning compared to pre-operative assessments.
Conclusion
Temporal nerve damage remains a significant concern in brow lift surgeries. Temporary neurapraxia is relatively common, while permanent damage is rare. Continuous monitoring and rehabilitation are essential for recovery. The case highlights the importance of meticulous surgical technique and post-operative care to enhance aesthetic outcomes and patient satisfaction. If facial nerve function does not improve, alternative treatments such as botulinum toxin injection or revision surgery may be considered. Awareness of potential complications is crucial for both surgeons and patients to manage expectations effectively.
