Purpose

Static brow lift surgery has seen renewed interest due to its ability to provide durable brow elevation but it is associated with visible eyebrow scarring. Given the prominence of the eyebrow region in facial aesthetics, techniques that restore hair-bearing skin may offer a means of improving scar concealment and patient satisfaction.

Methods

A patient presented with persistent aesthetic concerns following combined upper and lower blepharoplasty, facelift and brow lift performed abroad. Concerns included brow asymmetry, suboptimal position with a tethered, visible eyebrow scar. A three-stage revision protocol was implemented at three-month intervals: Stage I: Revision blepharoplasty to address eyelid malposition. Stage II: Revision static brow lift involving elliptical scar excision and repositioning, with careful preservation of the supraorbital and supratrochlear nerves. Stage III: After three months of scar maturation, follicular unit hair transplantation was performed under local anaesthesia to restore eyebrow density and camouflage the revision site.

Results

At follow-up after completion of the staged approach, improved brow height and contour were observed. The transplanted follicles demonstrated successful graft take with appropriate alignment to the natural eyebrow hair pattern, including satisfactory direction and density. Effective concealment of the surgical scar was achieved. The patient reported high satisfaction with the aesthetic outcome.

Adjunctive hair transplantation offers a valuable option for managing cicatricial alopecia following static brow lift surgery, enhancing aesthetic outcomes and patient satisfaction while revisiting a time-tested surgical technique.
