Prevalence of motility disorders in patients with cicatricial mucous membrane pemphigoid

Purpose
Mucous membrane pemphigoid (MMP) is a rare cicatrizing autoimmune disease. Ocular involvement can lead to fornix shortening, symblepharon formation and surface keratinization, resulting in blindness. While eyelid malpositions are known complications, ocular bulbus and eyelid motility have not been systematically investigated. This study aimed to investigate the prevalence and extent of these disorders and their correlation with Foster disease stage and ocular surface biomarkers.
Methods
In this prospective study, 40 eyes of 20 patients with ocular MMP were examined. Ocular motility was assessed using the Kestenbaum limbus test. Eyelid parameters (palpebral fissure, marginal reflex distance 1 (MRD 1), levator function, lagophthalmos, scleral show) and clinical biomarkers of the ocular surface were measured in a standardized manner. Statistical analyses included Spearman’s correlation, Kruskal-Wallis and Mann-Whitney U-tests.
Results
The prevalence of ocular motility disorders was 95% (38/40 eyes). Only 5% of patients reported diplopia. A significant correlation was found between the extent of ocular movement deficits and Foster stage (ρ = 0.67; p < 0.001). The prevalence of eyelid motility disorders or malpositions was 80% (32/40 eyes). MRD 1 and levator function showed significant negative correlations with the Foster stage (ρ = -0.53; p < 0.001 and ρ = -0.50; p = 0.001). No significant correlation was found between motility disorders and ocular surface biomarkers.
Conclusion
In this cohort, ocular and lid motility disorders as well as eyelid malpositions were highly prevalent and correlated with the disease stage according to Foster. These findings highlight the importance of early diagnosis and treatment of ocular MMP. The absence of correlation with ocular surface biomarkers suggests that ocular and lid motility, as well as eyelid malpositions, primarily reflect chronic structural scarring rather than acute inflammatory activity. In the future, ocular motility, eyelid motility, and malpositions could be integrated as functional biomarkers into extended staging systems for ocular MMP.
