Less Heat, Better Comfort? Blepharoplasty Under Local Anesthesia

Purpose
To evaluate the impact of electrocautery usage on patient comfort in blepharoplasty performed under local anesthesia, and to compare a conventional technique with a low-thermal selective bipolar and scissor-based approach.
Methods
This retrospective observational sequential study included patients undergoing upper and lower blepharoplasty under local anesthesia. Two surgical approaches were compared: a conventional electrocautery-based technique and a low-thermal technique using selective bipolar coagulation with minimal electrocautery and scissor-based tissue and fat excision. In my clinical practice, the low-thermal approach was adopted to reduce thermal tissue exposure. Intraoperative patient comfort, burning (thermal) sensation, patient cooperation, and early postoperative edema and ecchymosis were evaluated clinically.
Results
In our clinical experience, patients treated with the conventional electrocautery technique reported higher intraoperative burning sensation and reduced tolerance compared to the low-thermal group. The selective bipolar technique provided adequate hemostasis with minimal thermal energy use. The low-thermal group demonstrated improved intraoperative comfort, better patient cooperation, and comparable hemostatic control. Postoperative edema and ecchymosis appeared subjectively milder in this group.
Conclusion
Reducing thermal energy usage in blepharoplasty under local anesthesia may improve patient comfort without compromising surgical safety. A selective bipolar and scissor-based low-thermal approach appears to enhance intraoperative tolerance and overall patient experience.
