Ptosis Correction in Anophthalmic Sockets: Functional Outcomes and Determinants of Surgical Success

Purpose

To evaluate functional and aesthetic outcomes of ptosis correction in anophthalmic sockets and identify factors influencing surgical success.


Methods

Retrospective cohort study of 1400 eyes undergoing corneal tattooing between 1984 and 2024 at a tertiary center. Random selection was done for 303 eyes. Data on demographics, indications, pigment type, complications, retattooing, and satisfaction were analyzed. Procedures used a standardized anterior stromal puncture technique with either platinum chloride or Indian ink. Statistical analysis included χ² and t-tests with significance at p<0.05.


Results

MRD1 improved significantly by +2.47 ± 1.71 mm (p 1 mm was uncommon (3.8%). Patients requiring postoperative prosthesis modification demonstrated greater variability in MRD1 outcomes, supporting a dynamic interaction between prosthetic volume and eyelid position.


Conclusion

Ptosis correction in anophthalmic sockets achieves significant functional improvement with a low complication profile. However, outcomes are not solely determined by surgical technique. The interaction between prosthesis characteristics and eyelid mechanics emerges as a key determinant of both under- and overcorrection. Optimizing prosthetic fit alongside surgical planning may be critical to improving predictability in this complex population.
