Eyelid Squamous Cell Carcinoma: A Single-Surgeon 28-Case Series from a Tertiary Eye Care Center in Hungary

Purpose

To evaluate the clinical characteristics, surgical management, and outcomes of eyelid squamous cell carcinoma (SCC) treated by a single surgeon at a tertiary referral ophthalmology center.


Methods

A retrospective review was conducted of patients undergoing surgical excision of histologically confirmed eyelid SCC between January 2022 and January 2026 at the Department of Ophthalmology, Semmelweis University, Hungary. Demographic data, tumor characteristics, surgical approach, reconstructive techniques, margin status, and recurrence were analyzed. All procedures were performed by a single oculoplastic surgeon. Mean follow-up time was recorded.


Results

Twenty-eight patients were included (mean age 78 ± 11 years). Tumors were located on the right side in 57.1% and on the left in 42.9%. The most common locations were the lower eyelid and medial canthus (each 28.6%). Mean tumor size was 18.0 ± 16.5 mm. Tumor stages included in situ (21.4%), T1 (32.1%), T2 (28.6%), T3 (14.3%), and T4 (3.6%). Complete excision with histologically negative margins (R0) was achieved in 89.3% of cases; R1 resection occurred in 3.6%, while 2 cases were diagnostic biopsies. Reconstruction was most commonly performed using local flaps (75.0%), followed by direct closure (21.4%). Mean follow-up was 18 ± 28.6 months.


Conclusion

Eyelid SCC is a relatively rare but potentially aggressive periocular malignancy. In this series, most tumors presented at early to intermediate stages, with a high rate of complete excision. These findings provide insight into surgical management strategies and short- to mid-term outcomes in a tertiary care setting.
