Clinical lacrimal drainage system length among patients with congenital nasolacrimal duct obstruction

Purpose
This study aims to evaluate the intraoperative length of lacrimal drainage system (LDS) in patients with congenital nasolacrimal duct obstruction (CNLDO), and to develop a predictive model based on patient demographics to facilitate surgical management of CNLDO.
Methods
This retrospective cohort study was conducted at a tertiary medical center in southern Taiwan. The LDS length was measured intraoperatively in patients with CNLDO who underwent surgical treatment between September 2023 and March 2026. Demographic variables, including age, body weight, height and body mass index (BMI), were reviewed as predictors of the LDS length using simple linear regression analyses. Significant variables in simple linear regression models were subsequently included in the multiple linear regression models. Variables found to be significant were subsequently included in the multiple linear regression analyses as predictors.
Results
A total of 23 patients (46 eyes) were enrolled, with a mean age of 22.27 ± 12.50 months. The mean height was 81.70 ± 9.80 cm, weight 11.35 ± 3.13 kg, and BMI 16.72 ± 1.04 kg/m2. The mean LDS length was 33.28 ± 2.93 mm in the right eye and 33.63 ± 2.62 mm in the left eye. In simple linear regression analyses, age, height and weight were all significantly and positively correlated with LDS length in both eyes (p value all less than 0.001). Body weight showed the strongest correlation (R square= 0.737 for the right eye and 0.606 for the left eye). In multiple linear regression analyses, the best predictive performance was achieved when all three variables were included (R square= 0.764 for the right eye and 0.616 for the left eye).
Conclusion
The LDS length is positively correlated with age, height and especially body weight. This study improves the clinical understanding of LDS anatomy in CNLDO and may help optimize surgical technique and outcomes.
