Step-by-step Surgical Repair of Lower Eyelid Laceration with Inferior Canalicular Involvement Using a Monocanalicular Stent

Purpose

To demonstrate a systematic, step-by-step surgical approach for the primary repair of a traumatic lower eyelid laceration involving the inferior lacrimal canaliculus. The primary goal of this video is to illustrate the precise identification of the canalicular anatomy and the standardization of the reconstructive technique utilizing a monocanalicular stent to restore both lacrimal drainage function and eyelid margin integrity.


Methods

This video presents a 4-year-old male with a full-thickness lower eyelid laceration and transected inferior canaliculus. The procedure begins with wound inspection and probing to identify the canalicular cut ends. After inserting a monocanalicular stent (Mini-Monoka, FCI) into both segments, anastomosis is secured via anterior and posterior pericanalicular 6-0 Vicryl sutures. The video concludes with layered eyelid closure, ensuring proper margin alignment and tension-free skin closure.


Results

The video illustrates the complete anatomical reconstruction of the inferior lacrimal system and lower eyelid. It highlights critical maneuvers, like locating a retracted medial cut end and passing pericanalicular sutures without entering the lumen. The technique yielded excellent anatomical approximation, stent stability, and margin alignment, preventing complications like ectropion or epiphora.


Conclusion

Monocanalicular stenting with precise pericanalicular suturing and layered reconstruction provides an effective, safe method for managing canalicular lacerations. Standardizing these steps ensures optimal functional and aesthetic outcomes in adnexal trauma. This video serves as a practical educational tool for oculoplastic surgeons.
