Title Lower eyelid and medial canthal reconstruction using a Mustardé flap in periocular basal cell carcinoma: a case report

Purpose
To describe the reconstruction of a complex periocular defect following excision of basal cell carcinoma involving the lower eyelid and medial canthus.
Methods
An 81-year-old female presented with a 6-month history of an ulcerated lesion affecting the medial third of the left lower eyelid with extension to the palpebronasal region. Clinical diagnosis of basal cell carcinoma was made. Wide excision was performed followed by reconstruction using a Mustardé cheek rotation flap.
Results
Histopathological analysis confirmed basosquamous carcinoma with clear surgical margins. The flap provided adequate defect coverage with good tissue match. Functional outcomes included preservation of eyelid position and blink function, with no postoperative ectropion. Cosmetic outcome was satisfactory.
Conclusion
The Mustardé flap is a reliable technique for reconstruction of extensive lower eyelid defects involving the medial canthal region, offering favorable functional and aesthetic outcomes in complex periocular oncologic cases.
