Severe Cicatricial Entropion Following Orbital Floor Fracture Repair: Combined Palatal Mucosal Graft and Fat Pearl Augmentation with Differential Outcomes

Purpose
To describe the surgical management of severe cicatricial entropion following orbital floor fracture repair and to evaluate the outcomes of a combined approach addressing both posterior lamellar deficiency and orbital volume.
Methods
A patient presented with significant lower eyelid cicatricial entropion after prior orbital trauma and surgical repair. Surgical management included posterior lamellar reconstruction using a hard palate mucosal graft. In addition, autologous fat pearls were placed in the orbital floor region aiming to improve globe support and correct hypoglobus. Surgical technique and perioperative considerations are described.
Results
Postoperatively, the patient demonstrated satisfactory eyelid position with resolution of entropion and improvement of ocular surface symptoms. The palatal mucosal graft provided stable posterior lamellar support. However, fat pearl augmentation did not result in significant improvement of hypoglobus or orbital volume. No major complications were observed, and functional outcomes were maintained during follow-up.
Conclusion
Severe cicatricial entropion following orbital fracture repair represents a complex reconstructive challenge requiring a tailored approach. While palatal mucosal grafting proved effective in restoring eyelid position and function, fat pearl augmentation showed limited impact on orbital volume correction in this setting. This case highlights the importance of addressing both lamellar deficiency and post-traumatic orbital volume alterations, as well as recognizing the limitations of certain adjunctive techniques.
