The Effect of Hyaluronic Acid Filler on Corneal Topography and Tear Film Parameters in Patients with Superior Sulcus Deformity

Purpose
To evaluate the effects of hyaluronic acid filler for superior sulcus deformity on corneal topography and ocular surface parameters, and to explore the interaction between periocular soft tissue augmentation and anterior segment dynamics.
Methods
In this prospective single-center study, 20 patients undergoing hyaluronic acid filler injection and 26 healthy controls were included. The filler was placed in the suborbicularis plane using a cannula. Evaluations were performed preoperatively and at 1, 3, and 6 months, including Ocular Surface Disease Index, Margin Reflex Distance-1, corneal topography measured with Pentacam HR, Tear Break-up time, and Schirmer test.
Results
Pretarsal distance decreased and preseptal distance increased significantly at 1 month (p≤0.05), while Margin Reflex Distance-1 showed no significant change. Tear Break-up time decreased at 1 month (p=0.005), returned to baseline at 3 months, and increased at 6 months (p=0.009). Schirmer test values remained stable, and Ocular Surface Disease Index changes were statistically significant but clinically mild. Corneal topography was unchanged at 1 and 3 months. At 6 months, minimal flattening in K1, K2, and mean keratometry was observed (p≤0.05); however, these values were not significantly different from the control group. Kmax, astigmatism, and pachymetry remained stable.
Conclusion
Hyaluronic acid filler achieved effective anatomical correction without compromising ocular surface integrity or refractive stability. Stability of margin reflex distance-1 further supports a favorable safety profile. Corneal changes followed a low-amplitude, time-dependent, and reversible adaptive pattern without clinically significant iatrogenic astigmatism. For clinical practice, particularly in patients undergoing cataract surgery or premium intraocular lens implantation, careful timing and repetition of keratometric measurements are recommended to ensure accurate biometric planning.
